The statement that a batch was made under approved documentation — issued only when the batch was actually released and nothing failed.

No account needed. A batch that is not released produces a refusal rather than a certificate, and that is the point of the tool.

One per line. Anything listed here blocks the certificate — a document engine does not get to form its own opinion about a laboratory result.
